Please note that for your appointment you need to bring the following documents with you. Without them our Doctor will not be able to complete your D4 Form and you may need another appointment

Documents to bring for HGV / LGV / PCV / Ambulance and Motorhome drivers Medical

D4 Form Photographic identification (passport or driving licence)
Your glasses (if worn) Your most recent glasses prescription (if worn)
Your list of medications
Advised to bring a copy of medical history from your GP.

Documents to bring for PCO / Minicab / Taxi Medical

TPH/204 Medical Declaration Form (PCO applicants) or relevant
Medical Assessment Form to be completed
Photographic identification (passport or driving licence)
Your glasses (if worn) Your most recent glasses prescription (if worn)
Your list of medications Medical history from your GP
